In order to complete your tax returns, LPA Support will need various financial documentation. If you have previously had your taxes prepared by LPA Support, we can provide you a customized checklist as some of the documentation we will already have on file. New clients will need to present more documentation. For business tax returns, please contact us as the documentation required will vary. For personal tax returns, below is a checklist of the items most people will need. We recommend you print this list, attach it to a folder of your tax documents, and cross items off as you add them to the folder.
Personal Information
- Social Security numbers and dates of birth (You, your spouse, and your dependents)
- Copy of last year’s tax return
- Bank account and routing number or a voided check (for direct deposit of refund)
Income Information
- W-2 (You, your spouse, your dependents)
- 1099’s (You, your spouse, your dependents)
- 1099-C for canceled debt
- 1099-G for unemployment income, or state/local tax refunds
- 1099-MISC for any independent contractor work
- 1099-R, Form 8606 for payments/distributions from retirement plans
- 1099-S for income from sale of a property
- 1099-INT, -DIV, -B, or K-1s for investment or interest income
- SSA-1099 for Social Security benefits received
- Alimony received
- Business income (profit/loss statement)
- Rental property income/expenses (profit/loss statement)
- Miscellaneous income (e.g. gambling winnings, scholarships)

- 1098-E for student loan interest paid (or student loan statements)
